Childcare and School Vaccine Requirements

Make sure your child is up to date with vaccines. To beat the back to school rush, don’t wait until the end of summer. A good time to get vaccines is during a yearly well-child checkup.

Be sure to get an updated SC Certificate of Immunization for your school nurse. This certificate is required by state law for school attendance.

The Lancaster County Health Department also provides vaccines.

• Registration for New and Returning Students

Online registration should be completed for all new and returning students.

Register your LCSD student by visiting www.hoodini.school

It is important to notify your child’s school nurse of any illness or condition that your child has that may require medical support during the school day.

• Medication

If your child will need prescription medication during the school day, the Permission for School Administration of Medication form must be completed and signed by your child’s doctor.

Over-the-counter medications (Ibuprofen, Tylenol) needed during the school day must be provided by the parent. The parent must complete and sign the Permission for School Administration of Medication form.

New medication forms are required at the beginning of each school year.

• Self-medicating/Self-monitoring

• Special Dietary Accommodations

A Diet Order is required for students who need special dietary accommodations. This form must be

completed and signed by a health care provider.
