The first day of 4K screening will be Monday, February 19. In order for a child to be screened, all required registration documents must be turned into the child's home school BEFORE the screening date.
Bring a copy of the following documents to your local school: –Parent/Guardian Photo ID – Birth Certificate (child must be 4 by September 1, 2024) – Immunization Record – 2 Proofs of Residency – Proof of Income Eligibility (child’s Medicaid card, household W-2, or pay stub)
