ID Badge Replacement

When an employee, vendor, or non-employee (‘Cardholder’) loses or has an ID badge stolen:

1.      The Cardholder must immediately report the loss or theft to Human Resources by calling 803-416-8953 or emailing petrina.boulware@lcsd.k12.sc.us.

2.      The Cardholder must report to the Bookkeeper at their school site or to the designated cash collection point at other locations to pay a replacement fee for the lost or stolen access badge.

3.      The Bookkeeper will forward the payment to Human Resources at the District Office.

4.      Human Resources will issue a new badge and send it, along with a payment receipt, via the district courier to the Cardholder’s site.

When an employee, vendor, or non-employee (‘Cardholder’) has a broken or defective badge:

1.      The Cardholder must email petrina.boulware@lcsd.k12.sc.us, describing the issue with the badge (e.g., broken or malfunctioning) and include a photo of the back of the badge.

2.      If the badge is deemed beyond repair, Human Resources will issue a replacement badge and send it through the district courier.

 

It is the Cardholder’s responsibility to keep their ID badge safe and secure at all times. Loss, theft, or damage due to negligence may result in required replacement fees, as well as further actions if not reported to Human Resources immediately.
